Senior Transition Program
Flow Chart of Activities
Students are provided information about the Hinds County Senior Transition Program through presentations from Transition Coordinators from DREAM, Inc., school counselors, announcements, flyers, brochures, www.transitions.ms , and other communication channels.
If students are interested, they complete a pre-application packet which includes a few simple forms.
Students are eligible if they are high school seniors on track to graduate in the Spring, meet certain income guidelines and have any work readiness challenges
Transition Coordinators collect Preliminary Applications with supporting documentation such as a birth certificate, social security card and Work Readiness assessments from prospective student participants and document initial eligibility.
Transition Coordinators notify students of eligibility, schedule an orientation to the program and a visit to WIN center to learn how to search for jobs and other workforce and educational resources.
Transition Coordinators complete Individual Career Plans with students, schedule appropriate workshops, and help students develop a portfolio for their work.Transition Coordinators ensure that workshops take place on schedule, verify attendance, “grade” individual workshop outputs, document in case files and include in individual portfolios.
Transition Coordinators confirm students’ successful completion of program and provide them with a $250 program incentive.
Students are exited from program.
Transition Coordinators conduct follow-up on all participants
